WHILE THE ORGANIZERS WILL STRIVE TO ACCOMMODATE AS MANY ARTISTS AS POSSIBLE, WE CANNOT GUARANTEE THAT ALL APPLICATIONS WILL BE AWARDED A BOOTH SPACE DUE TO THE LIMITED NUMBER AVAILABLE.

All artists must supply
their own lighting and display materials.

Lights must be white halogen
or regular incandescent white bulbs.

No Christmas lights or
decorations are allowed.

Depending on booth size
total wattage of lights in each booth space varies.

I am applying to enter the Originals Only Fine Art Show & Sale
to be held Saturday and Sunday,
November 14th and 15th 2009, in the Comox Recreation Centre, and will adhere to the conditions of
rules.
My total fees
(booth charge plus GST), (cheque
payable to the Town of Comox) are submitted with this application; and will be refunded in whole if I am not able to be assigned a booth.

Originals Only Art Show & Sale Liability
Waiver
This waiver
must
be signed for each show so please sign and remit with application.
With my acceptance as a participant in the
Originals Only Art Show & Sale or any activity associated with it, I agree
to release the Comox Recreation Commission, The Town of Comox and the
Organizing Committee of the Originals Only Art Show & Sale and/or their
employees, agents or volunteers from and against all claims, actions,
costs, expenses and demands in respect of death, loss, injury or damage to
my person or property whatsoever, caused, arising out of or in connection
with my taking part in the event and notwithstanding that the same may
have been contributed to or occasioned by any act or failure to act
(including without limitation, negligence) of members of the Originals
Only Group. The Comox Recreation Commission, The Town of Comox
and/or any one or more of their employees, agents or volunteers.
This agreement is binding on my, my heirs, executors and administrators.
